Billing Arrangements

We are a private billing practice. Information about our fees and services are available at the Reception.

A small fee is charged for procedures, dressings and vaccinations.

We do bulk bill Pensioners and Health Card Holders who bring a current entitlement card, and children below 16 years of age. This is during week day normal hours (9.30am to 3.45pm) appointments.

Special Practice Notes

Patients are required to call for an appointment directly to the practice regarding:

Bone Density Screening, Iron Infusion, Immunisations/Vaccinations, Insurance Assessment, Driving Assessment, Ear Syringing, Health Assessment, Care Plan and any Procedural Consultations.

As you will need an appointment to see both the Doctor and the Nurse.

Referrals: Doctors in this practice may if required refer you to a specialist for further investigations. You can discuss this with your doctor including potential out of pocket expenses.

Test Results: Results are reviewed by your doctor and acted on in a timely manner. We will contact you if necessary.

Communication: A doctor is available during our normal opening hours for emergency advice. Our staff are experienced in deciding the appropriate response to any phone request.

Appointment times: We endeavour to see patients on time every time, except if we are faced with an emergency situation. Our reception will inform patients if we are running late.


Consultation is by appointment. You can book via Health Engine by clicking the BOOK ONLINE button above or call us on 9452 9999. If you call us after we are closed please leave us a message and we will call you back.

Urgent cases are seen immediately. Give us a call if you are on the way to our clinic so we can be prepared for you.

Booking a long appointment is important for complex health matters or insurance medical, health assessments, counselling, a second opinion etc. If you require a long appointment, please call us and ensure you bring along all relevant information.

If for any reason you are unable to keep an appointment and wish to cancel or running late, kindly give us a call.

If more than one person needs to see a doctor, kindly inform reception when making the appointment.

New patients, please arrive 15 minutes prior to your appointment time to complete the patient registration. We will also require a valid Drivers License, Passport and Medicare Card.

We require a minimum of 2 hours for any cancellation or rescheduling of an appointment to avoid the fees.


Dr Praba Selvendra


After completing her medical training, Dr Praba  worked in Fremantle Hospital and Royal Perth Hospital for several years where she gained diverse knowledge in all aspects of patient care. Dr Praba enjoys her chosen profession as a GP and has  special interests in children and adolescent medicine, antenatal care  and mental health. She has vast experience in travel medicine, chronic disease management and all aspects of women's health including contraception advice.

Dr Sanjeev Balakrishnan

Sanjeev holds a Master of Medicine in Family Medicine from the National University of Singapore.  He is very passionate about management of obesity, type 2 diabetes and metabolic syndrome through healthy eating. He is an advocate for the scientifically proven low carbohydrate-high healthy fat diet*

Sanjeev is a GP with almost two decades of experience and besides general practice, his special interests are in men's health, family planning and the management of STIs. He also performs minor procedures such skin excisions, skin biopsies and insertion & removal of Implanon. 

He is a fully credentialed Family Physician in Singapore and is a Fellow of the Royal Australian College of General Practitioners.

Dr Sanjeev works at Park Medical on Mondays , Wednesdays, Fridays  from 2pm to 7pm and on alternate Sundays from 9am to 11.30am.


Dr Latha Kumar (MBBS, FRACGP)


Dr Latha Kumar (MBBS, FRACGP) graduated in 2000 from India. She did her postgraduate training in various tertiary hospitals including Sir Charles Gairdner, Hollywood Private, Princess Margaret Hospital, Joondalup Health Campus, Osborne Park Hospital and SJOG, Subiaco before she gained Fellowship of Royal Australian College of General Practitioners in 2012(WA).

She enjoys all aspects of Family medicine, Women’s Health, Men’s Health, Paediatrics, Chronic disease management, Mental health and minor surgical procedures. She believes in preventative health and holistic approach as part of her management plans. She is married and mother of 2 active boys. When not at work, she enjoys cooking, travelling and spending time with family and friends.

She is available for consultation from Mondays to Fridays and one Saturday in a month.

She speaks English, Kannada, Hindi and Tamil.

Dr Wern Hui Yong


Dr Yong graduated from the University of New South Wales. She has worked in multiple areas of Medicine in Singapore and Australia. Prior to General Practice, she trained as a rheumatologist in Singapore. She enjoys the broad range of medicine in general practice, particularly women's and children's health.

Dr Jaspreet Mudhar


Women’s’ and children’s health and family planning counseling, minor surgical procedures, chronic disease management and  Implanon insertion and removal.

Dr Sarat Jyotsna Rangaiah


Dr. Jo is a local graduate who completed both her undergraduate studies at the University of Western Australia and her post graduate training in General Practice here in WA.  Dr Jo has worked at major tertiary hospitals including Fremantle Hospital and Princess Margaret Hospital. She enjoys the diversity of general practice and is interested in all aspects of general practice, particularly women’s health and children’s health and mental health. Dr Jo is also qualified in Implanon insertion and removal and is able to do some minor surgical procedures as well.

Dr Nick Smith


Dr Nick Smith graduated in 1982 and worked in England before coming to Australia in 1996. Since then he has worked in Western Australia area with interests in aged care and men’s health. Dr Nick works Fridays 9am to 4.00pm

Dr Niroshi Susiriwardana 


Dr Niroshi graduated from Sri Lanka in 1998 and since then has worked in many fields such as General Surgery,  General Medicine,  Paediatrics,  General Practice and Anaesthetics until 2011.

After arriving in Australia in 2011,  she started working as a Rural Generalist giving her the opportunity to practice in multiple disciplines such as Anaesthesia, Emergency Medicine, General Medicine and Surgery, Paediatrics, outreach clinics as well as General Practice. 

She is a Fellow of Royal Australian College of General Practice and has vast experience in Chronic Disease Management, Acute Medicine, Emergency Medicine, Anaesthetics, Minor Surgeries and Dermatology.

Dr Niroshi has a special interest in aging skin and skin rejuvenation and also performs skin cancer checks.

Dr Hermant Garg

Hemant graduated from the University of Western Australia in 2008 and he has worked all over WA, from Broome to Mandurah and in the major tertiary hospitals in Perth.

He brings a breadth of experience to general practice, including time in general and orthopaedic surgical units as well as many years of Emergency medicine experience covering all ages from infants to the elderly.

Hemant has a special interest in metabolic health and the treatment of obesity and diabetes. He is a proponent of the low carbohydrate diet for these conditions (as well as having practised this diet himself for the better part of the past two years).

Hemant is a firm believer of the principle of shared decision making where Doctor and Patient work together to develop an understanding of the problem and come up with a management plan that is most likely to work with the patient’s needs and lifestyle.

Dr Arwen Boyle (GP Registrar)

After completing her medical training at UWA, Dr Boyle has worked all over Western Australia from Broome to Bunbury gaining broad experience looking after her patients in hospital, emergency and surgical settings. In Perth, she has worked at Royal Perth Hospital, St John of God Subiaco, Sir Charles Gairdner Hospital, Princess Margaret Hospital, Armadale-Kelmscott Memorial Hospital and Joondalup Health Campus.

Dr Boyle has extensive surgical experience which she will bring to the practice at Park Medical Group.

Dr Boyle is interested in all areas of Family medicine with a special interest in supporting women in the perinatal period, particularly with breast feeding, as well as surgical procedures.
