An over-75 health assessment is a free, annual Medicare-funded check-up in Australia for older adults, designed by your GP and practice nurse to comprehensively review your medical, physical, psychological, and social well-being, identifying health risks, promoting independence, managing medications, and creating a personalized care plan to support healthy ageing and reduce hospital visits. It covers everything from falls risk, vision, hearing, and mental health to lifestyle factors like nutrition and exercise.

What it involves?

Medical Review- Medications (polypharmacy risks), chronic conditions, immunisation status.

Physical Function- Mobility, balance, continence, falls risk, vision, hearing.

Cognitive & Mental Health- Memory, mood, social engagement, cognitive screening.

Social & Living Situation- Support networks, home safety, care needs, transport.

Health Promotion- Advice on diet, exercise, and healthy living.

Care Planning- Development or update of a personalised care plan with potential referrals.
