An over-75 health assessment is a free, annual Medicare-funded check-up in Australia for older adults, designed by your GP and practice nurse to comprehensively review your medical, physical, psychological, and social well-being, identifying health risks, promoting independence, managing medications, and creating a personalized care plan to support healthy ageing and reduce hospital visits.
